Ready to make an allegation?

Here are some important things to keep in mind before reporting your allegation to us.

  1. Completing this form does not automatically result in FACE opening an investigation. This form is confidential and only official FACE representatives that are part of the investigative team will see it.

  2. Any information shared in this submission will be saved indefinitely in our records and can not be redacted or retracted.

  3. You are welcome to submit an anonymous allegation but please understand we cannot take action on an anonymous allegation. FACE has to be aware of your identity and have a valid form of contact for you to proceed.

  4. At this time FACE is only equipped to investigate cases in the United States and Canada. You may submit an allegation that originates in another location and we’d be happy to provide guidance to the best of our ability but unfortunately we do not have the agency or resources to investigate it.
